SMS Terms of Service
Last Updated: October 2026 | These terms apply to text messages sent by Fresno Long-term Care Medical Group (FLTCMG), 5150 N 6th Street #149, Fresno, CA 93710.
1. Program Description
When you opt in, Fresno Long-term Care Medical Group may send you text messages about appointments and house-call visits, care coordination, and replies to messages you send us. You may also text our office and we will reply. We do not send marketing or promotional text messages.
You can opt in by checking the text message consent box on a form on our website, or by texting our office first. Consent to receive text messages is not a condition of receiving care.
2. How to Opt Out
You can cancel the SMS service at any time. Just text STOP to the number that messaged you. After you send STOP, we will send you one text message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time, or text START, and we will start sending SMS messages to you again.
3. Help
If you are experiencing issues with the messaging program, reply with the keyword HELP for more assistance, or get help directly by calling (559) 222-6030 or emailing Info@fltcmg.com.
4. Carrier Liability
Carriers are not liable for delayed or undelivered messages.
5. Message Frequency and Rates
Message and data rates may apply for any messages sent to you from us and to us from you. Message frequency varies based on your appointments and conversations with our office. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
6. Privacy
Your mobile number and consent are never shared or sold to third parties for marketing purposes. If you have any questions regarding privacy, please read our Privacy Policy.
Medical emergencies: Do not use text messages for emergencies — call 911. Please do not text sensitive health information such as Social Security numbers or detailed medical history.
